How about a Shadowrun negative quality?  Haven’t thought up one of those in a long time….

Bonus: 10 Karma

Be it surviving a fire and not getting treatment to just being a long time smoker, you’ve burnt your lungs pretty hard.  Increase the difficulty to overcome all inhaled poisons, toxins, or other effects by +2.
